De DEA agent Max Payne (Mark Wahlberg) wil, nadat zijn vrouw en kind zijn vermoord, wraak nemen op de schuldigen. Tijdens een zaak wordt Payne echter vals beschuldigd van moord en wordt hij op de hielen gezeten door zowel de politie als door de verantwoordelijke criminele organisatie. Hij wordt hierbij vergezeld door Mona Sax (Mila Kunis), omdat zij wraak wil op de moord van haar zus.
